55 year old female patient, a resident of Miryalaguda and farmer by occupation came to the hospital on 17/5/21 with the chief complaints of shortness of breath, pedal edema and facial puffiness.
EVOLUTION OF SYMPTOMATOLOGY-
- First episode 20 years ago lasted for 1 week and occured in the month of january that year. (during which time she was working in a paddy field. This first episode was relieved upon taking medication)
- Next 8 years the patient has suffered from similar episodes of SOB every year each lasting approximately 1 week and occurring around January (when she worked at the paddy fields) these episodes were all relieved upon taking medication.
- 12 years ago she had another episode of SOB.one lasted 20 days and she had to be hospitalized.
past 12 years she has been having an yearly episodes now lasting around a month again around january. Until her latest episode the SOB was of grade II
- latest episode of shortness of breath started 30 days ago
OTHER SYMPTOMS-
5 yrs ago-Anaemia
1 month back-Generalised weakness
20 days ago- Hypertension
15 days ago- pitting type of pedal edema and facial puffiness

PRIMARY ETIOLOGY OF THE PROBLEM-
HERE THE ALLERGEN IN THIS CASE IS THE RICE DUST.
THIS ALLERGEN TRIGGERS THE LYMPHOID TISSUE AND THE PLASMA CELLS TO RELEASE THE IgE WHICH CAUSES THE IgE TO BIND TO THE MASTCELLS AND RELEASES THE HISTMINE AND BRADYKININ.THIS CAUSE THE NARROWING OF THE RESPIRATORY TRACT BY INCREASING THE MUCOUS PRODUCTION .

But I feel it would be better in offline when compared. While we cannot attend hospitals in this COVID pandemic situation,learning about medicine thruogh patient based online learning,makes us keen to know more about different topics related to patient disease and condition,it will also makes us to search about a topic in google and made us to read about various literature and research work around the world.
We maynot show interest everytime,but in majority times,we are getting interested to know and to learn about new topics and will gain practical and applicable knowledge
